Lots of other possibilities: an author senior to the referee could do all
sorts of unpleasant things to the referee's career.
(the first paper I ever refereed was for a very good journal when I was a
grad student. The author of the paper was a young lecturer at the
university where I was soon to apply for a job. All sorts of things could
have gone wrong but did not. We had independently discovered the same
thing in different contexts and submitted to the same editor of the same
journal. I found out much later we had refereed each other's work. Both
got published after minor amendments. I still think many years later the
editor picked suitable referees for both papers. We published a joint
paper some years later on a further development.)
