In my statistics classes, I allow the use of a 3"x3" Post It. The student may write
ANYTHING on the Post It as long as the Post It will stick, without the aid of tape
or some such device, to the second page of the test (it could get brushed off of
the first page and I teach more than one section so I don't want notes passed to
the second class). I pass out the Post It so that I know that the size is that
required.

Like Anne, I have found that it does not change the grade disribution but it does
relieve test (and math?) anxiety.

I do not do this for upper level courses, at least not yet.
